The Certified Nurse Aide I will acquire and demonstrate a variety of nursing skills such as taking vital signs, communicating with patients and other healthcare team members, and assisting patients with activities of daily living. This team member is a vital part of the health care team.

Requirements for Enrollment for any course offered at Chosen Healthcare (Lumberton Location):

• Two forms of ID i.e. social security card, Drivers license ID, Passport, or Military ID
• High School Transcript or GED Transcript
• Criminal back ground check can be completed at Chosen Health Care Services for $20.
• Physical Exam completed at clinic or Physician’s office
• TB test can be completed at Chosen Health Care Services for $20

Nurse Aide I (Refresher)

This course does not replace the CNA I original class. Candidate must be CNA in another state or have expired CNA in North Carolina. A print out of a CNA I license in another state or expired license in North Carolina is required.

This refresher course will reintroduce the varieties of Nursing skills needed to assist patients with activities of daily living. Upon completion, students will be refreshed in how the Nurse Aide works.

Requirements for Enrollment for any course offered at Chosen Health Care Services:

• Must be 18 years or older
• Two forms of ID i.e. social security card, Drivers license, Passport, or Military ID
• High School Transcript or GED Transcript
• Physical Exam completed at clinic or Physician’s office
• TB test can be completed at Chosen Health Care Services for $20
• For Pharmacy Tech, additional requirement is a Drug Test
• Criminal back ground check can be completed at Chosen Health Care Services for $20.

Duration of the class: 1 week ( Monday to Friday or 5 consecutive Saturdays)

Nurse Aide II

The CNA II personnel will demonstrate the same skills as the CNA I with the addition of other advanced skills such as oxygen therapy, removal of fecal impaction, sterile dressing change, suction, and trach care. Students wishing to complete the CNA II course MUST have their CNA certification before entrance into the course.

Requirements for Enrollment for any course offered at Chosen Healthcare:

• Must be 18 years or older
• Two forms of ID i.e. social security card, Drivers license ID, Passport, or Military ID
• High School Transcript or GED Transcript
• Physical Exam completed at clinic or Physician’s office
• TB test can be completed at Chosen Health Care Services for $20
• Must be a nursing assistant 1 before taking the CNA II class
• Criminal back ground check can be completed at Chosen Health Care Services for $20.

Phlebotomists are medical professionals who draw blood from patients for various lab tests and procedures. Major course objectives include: basic terminology, basic anatomy and physiology, mastering proper needle techniques, and infection and safety precautions. We offer the opportunity to become a phlebotomist through a 160 hr 8 weeks course where students will be educated on the skills to become a safe phlebotomist.

Upon Completion of the Course & Clinical. Students will be eligible to take NHA Exam either at Chosen Healthcare Institute Inc, or at the PSI testing center.
